I understand and authorize that HealthPro Innovation may contact my educational institution(s), the NSC DegreeVerify or DiplomaVerify service, registrar offices, and licensing boards to confirm the information I have provided, including: institution name(s), degree type(s), field(s) of study, enrollment start date(s), enrollment end date(s), degree conferral/graduation date(s), enrollment status, degree award status, and professional license/certification details.
This authorization is provided as a standalone document separate from my employment application, as by the FCRA (15 U.S.C. § 1681b(b)(2)(A)) and the Illinois Human Rights Act (775 ILCS 5). I understand my information is protected under the Family Educational Rights and Privacy Act (FERPA) and will be released only pursuant to this signed authorization.
I have been provided with the CFPB "A Summary of Your Rights Under the Fair Credit Reporting Act" (March 2024 version). I understand I have the right to dispute inaccurate information and that no adverse action will be taken without proper pre-adverse action notice and a waiting period. This authorization is valid for 90 days from the date of signature.
